Sr Clinical Therapist

Summary:

Provides crisis intervention and symptom stabilization
emergency evaluation care coordination consultation and education to
consumers of all ages who present with behavioral health needs. Services will
be coordinated with family and other professionals or concerned parties. Conduct
substance abuse treatment with clients in need according to the 12 core
functions of substance abuse counseling as defined by the �ICRC.�

 

Responsibilities:

Provides
Screening Evaluation on the Telephone and Face to Face.



 



Provides emergency assessments to consumers.



 



Ability to use assessment tools for proper
diagnosis.



 



Assess consumers for risk of harm to self or others.



 



Recommends disposition and plan for stabilization
and offer treatment options and recommendations that are least restrictive
alternatives appropriate to level of risk. 



 



Consults with all relevant parties to seek out
necessary clinical information. 



 



Facilitates involuntary/voluntary hospitalizations
of consumers at risk while at the ER and also in the community.



 



Provides emergency case management for consumers
until they are no longer in crisis.



 



Identifies and offers referral assistance and
coordination to consumers who do not meet criteria of Gateway Healthcare Inc.
treatment programs.



 



Schedules and participates in psychiatric/
medication evaluations with psychiatrist as needed.



 



Evaluates medication side effects and reports
results to psychiatrist and RN as needed.



Possesses current job knowledge of all agency
programs their functions and is able to access these programs effectively.



 



Coordinates and consults with ER staff while
performing an intervention and acts as liaison between the hospital and
referral resources. 



 



Ensures
that contact with consumers involved in the agency is communicated to the
primary clinician and physician.



 



Consults
with all relevant parties to seek out necessary clinical information. 



 



Provides emergency
services at the consumer�s home and in the community in coordination with
others such as peers police hospitals and social service or medical agencies.



 



Participates in treatment planning meetings and
difficult case conferences when necessary for specific consumers.



 



Initiates calls or home visits to monitor a
consumer�s progress and revises the stabilization plan as needed.



 



Available
by telephone to discuss treatment issues monitor consumer�s response to
treatment and provide support and direction.



 



Adheres to documentation standards in accordance
with Gateway Healthcare Inc. policy and procedure in accordance to program standards.



 



Documentation of clinical work submitted according
to program guidelines.



 



Obtains past records in order to substantiate and/or
support diagnoses for treatment.



 



Obtains proper authorization from insurers as
needed.



 



Has consumer complete and sign GHI insurance forms
or seeks out other appropriate signatures when the client is unable to sign.



 



Functions as a resource for clients ER and Gateway
staff about behavioral health care issues and resources through consultation
and educational activities.



 



While at the ER provides education for ER staff as
needed for behavioral care issues.



Rotates with other clinicians the duties of
conducting telephone screenings and face to face assessments.



 



Provides intensive services to active ES clients
while working cooperatively with the primary clinician.



 



Participates in treatment plan meetings when
necessary for specific consumers.



 



Schedules duties in coordination with other ES team
members and modifies work schedule as needed in order to ensure that the department
is able to be responsive to consumers needs.



 



Appropriately
responds to incoming calls on National Suicide Prevention Lifeline by actively
engaging the caller while also functioning as a resource to the caller:
clinician will assess for risk coordinate referrals and/or emergency services
and completing appropriate documentation of each call.



 



Is reliable
and available by pager and/or by phone during the performance of duties.



 



Will
respond to all requests for consultations/evaluations by the staff in other
departments and/or from our community partners (i.e. local medical providers
family police) and will assist as needed in coverage at other ES locations if
staffing necessitates and/or supervisor�s discretion.



 



Will have
final decision-making responsibility for involuntary hospitalization and will
ensure all alternatives are explored and ruled out.



 



Will take
full responsibility in handling requests for services from the GHI Community
Disaster Team. This includes assessing the need for a team response calling
team members to organize and activate a response and notifying management
according to Disaster Team Response Policy.



 



Will
provide direct service backup coverage to other ES staff as needed. 



 



Participates in all necessary meetings as required. 



 



Performs other duties as assigned.

QUALIFICATION REQUIREMENTS:



Masters
degree (MA) MSW M.Ed. LCSW or LICSW preferred. Must be eligible for
�Qualified Mental Health Professional� status. 


Effectively present information and respond to questions from professionals managers clients family and the general public.

Read analyze and interpret professional journals policy and procedures and governmental regulations.

Write reports and general correspondence.

Analyze client and system information identify problems establish facts draw valid conclusions develop options and strategies for resolving problems monitor implementation and evaluate the problem solving process for effectiveness.

Deal with several abstract and concrete variables concurrently.

Ability to successfully complete all required training and certifications

Oral and written communication skills including speech grammar spelling and legibility are at sufficient level to ensure effective and appropriate communication.

Ability to add subtract multiply and divide in all unites of measure using whole numbers common fractions and decimals either manually or by use of a calculator.

Uses and accesses information in client�s health information record or about a client only as necessary to actual duties and then only in accordance with access levels assigned to this position by Gateway Healthcare Inc. from time-to-time and in accordance with Gateway Healthcare Inc. policies rules and procedures as adopted by Gateway Healthcare Inc. from time-to-time.
