Responsibilities: PRINCIPAL DUTIES AND RESPONSIBILITIES: Adheres to the policies and procedures of the Home Infusion Patient Management Program (HIPMP) and encourages patient participation in the Program. Participates in the development and execution of patient Plans of Care and the evaluation of patient progress towards goals. Provides written education materials and verbal consultation as needed to patients and/or caregivers relative to prescribed home infusion Medication. Ensures relevant patient specific parameters are included on the patient�s medication profile (e.g. diagnosis known allergies weight age etc.). Maintains working knowledge of current compounding and sterile preparation principles stability and compatibility (i.e. USP <797> and USP <800>). Maintains compliance with regulatory boards and regulations related to compounding of sterile pharmaceuticals [i.e. RI DoH RI Board of Pharmacy Rules and Regulations USP <797> and USP <800>]. Maintains knowledge of current requisites for safe handling administration and disposal of hazardous drugs. Provides detailed information and guidance to assist with effective treatment of extravasations. Accesses safety data sheets (SDS) and reacts appropriately to hazardous medication exposures or spills. Reviews each order or prescription to determine therapeutic appropriateness of the patient�s drug regimen as it relates to the patient�s clinical status concomitant drug therapy and diet. Reviews enters and verifies orders for medications including chemotherapy for appropriateness of drug dosage formulation route of administration duration of therapy and potential incompatibilities. Reviews enters and verifies orders for medication-related supplies for appropriateness and completeness. Identifies recommends communicates and clarifies with prescriber appropriate changes in therapy and/or clarification of unclear orders. Performs computer activities necessary for compounding. Understands and appropriately follows requirements for product documentation and check processes. Completes accurate documentation of finished compounded medications per policy. Performs detailed in-process medication checks for appropriate drug diluent diluent volume drug volume base solution base solution volume beyond use date storage requirements and labeling. Provides input on safety events related to home infusion services to minimize risk of reoccurrence. Uses appropriate health literacy skills to provide effective patient counseling on treatment regimens including precautions to follow and potential adverse reactions. Provides oversight to and mentors technicians residents and students. Shares expertise by participating in the orientation and training of pharmacists residents technicians and students. Maintains appropriate documentation and retains records regarding drug purchasing storage preparation and distribution in accordance with established policies and procedures and existing federal and state rules and regulations. Utilizes reporting and inventory management systems effectively in applicable pharmacy systems. Collaborates with pharmacy purchasing teams to procure medications and related supplies in a timely manner. Participates in continuous quality improvement initiatives related to home infusion services. Maintains professional competence through participation in Pharmacy staff development and skills enhancement programs including but not limited to ongoing independent study and membership and participation in professional organizations. Performs other duties as assigned.
Other information: BASIC KNOWLEDGE: Bachelor of Science in Pharmacy or Doctor of Pharmacy degree from an accredited college of pharmacy. EXPERIENCE: Effective interpersonal and communication skills both written and verbal to interact appropriately with patients families colleagues and others. Effective organizational and presentation skills. Knowledge of area/specialty specific technology including but not limited to pharmacy information systems automated dispensing technology software equipment and automation. Experience with MS Word Excel Outlook and web browsers. Must have the ability to multi-task and reprioritize as needed while maintaining accuracy and service standards. Must obtain within 2 months of start date and maintain no less than semi-annually thereafter internal home infusion services credentialing as demonstrated by successful completion of current competency assessments. WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS: Extended periods of time spent standing and walking. Requires the visual and manual dexterity to operate a computer. INDEPENDENT ACTION: Functions independently within broad scope of departmental policies and practices. Refers specific complex problems to supervisor when clarification of departmental policies and procedures may be required. SUPERVISORY RESPONSIBILITY: None.
