Summary: Under the supervision of the Manager of Patient and Family Centered Care the Bereavement Coordinator as a member of the Patient and Family Centered Care team. Provides grief support for families both inpatient and outpatient. Works directly with the Hasbro Bereavement Committee and serves as a liaison between department staff and families. They will support families and facilitate bereavement programs to enhance Patient and Family Centered care throughout the grieving journey. . Participates in quality initiatives throughout the hospital system.
Responsibilities: Primary Responsibilities: Provides direct services such as grief support programming resources and education to bereaved families. When requested by the health care team will provides initial consultation for end-of-life and bereaved families. Provides ongoing support in assisting families with coping with the death of a child Functions as a Bereavement Coordinator within role as a member of pediatric team. Promotes and facilitates communication between Bereavement Committee hospital staff and families. Provides ongoing education/resources regarding end-of-life and grief. Work with the Hasbro Bereavement Committee to assist in annual memorial service Funeral Assistance Fund grief groups Memorabilia Website/Resources and Representatives to support the needs of patients and families. Facilitate the Family Advisory Council Subcommittee Bereavement group as well as focus groups as needed to assess current programing and review additional options to embrace the needs of our grieving families. Works in collaboration with the Hasbro Bereavement Committee CHIPS and our interdisciplinary healthcare providers. Manages and oversees grief support programs and bereavement activities Creates annual grief group calendar including identifying group facilitators Provides planning and oversight for all functions of the grief support programs (brochures website flyers etc.) High level of interpersonal skills to function as a member of an interdisciplinary team and to effectively interact independently with patients families and others within and outside the hospital.? Procure program supplies monitor inventories and track program expenses. Demonstrates community engagement through events and educational presentations to increase awareness and access to grief support services for the entire community Excellent oral and written communication skills required.? Ensures that all program policies and procedures are adhered to Embraces and appreciates diverse Spanish speaking cultures; Spanish Speaking preferred Strong organizational problem solving and negotiation skills.? Ability to function independently as a Bereavement Coordinator Maintain confidentiality of all patients and family members.?
Other information: EXPERIENCE/EDUCATION: Bachelor�s degree required in Social Work or similar degree. Master�s degree preferred. Certification in Thanatology is desired but not required. ? WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS:? Normal office environment with some exposure to infectious diseases. Hazards are minimized by adherence to established safety precautions.? Considerable walking to meet with patients and families at various locations within the department as well as throughout the hospital setting.? SUPERVISORY RESPONSIBILITY:? None?
