Clinical Documentation Assistant

SUMMARY

Assists providers with having accurate patient charts prepared prior to the scheduled office encounter to ensure a timely and quality patient encounter.

Expert handling of patient issues with excellent customer service skills and patient confidentiality are mandatory.

PRINCIPAL DUTIES AND RESPONSIBILITIES

  • Responsible for preparing accurate and detailed health information in the electronic health record to the fullest extent prior to patient office encounter.
  • Works directly with the providers to maximize efficiency and optimize patient throughput by tracking laboratory and radiographic test results and transcribing them into the medical record, retrieving prior patient records including historical laboratory and radiology test results, referring physician office records and hospital discharge summaries.
  • Ensuring the timely weekly preparation of patient electronic health records for each assigned provider's clinical office sessions.
  • Attention to detail and critical thinking essential to the position to ensure a quality product.

EDUCATION

  • High school diploma or equivalent

EXPERIENCE/SKILLS

  • Prior medical chart and transcribing experience
  • Knowledge of medical office procedures and terminology
  • Confidentiality and privacy of the patients
  • Proficient in relevant computer applications and electronic medical record
  • Excellent data entry and typing skills (50+ ppm)
  • Excellent listening, verbal, and written communication skills
  • Ability to work independently and consistently deliver high quality work
  • Ability to read, understand and follow oral and written instructions given in English
  • Ability to establish and maintain effective working relationships with customers (patients, physicians, coworkers, supervisor, etc.)
  • Ability to represent the organization and serve consumers in a professional manner and promote a positive image of the organization and its services
  • Interpersonal skills necessary to deal effectively with patients, their representatives, and other personnel/coworkers
