Rehabilitation Liaison PD

Summary:

Under the direction of the

Director of Inpatient Rehabilitation Services the Rehabilitation Liaison

performs off-site preadmission screenings promotes the services of Brown University Health�s inpatient rehabilitation facility (IRF) and facilitates appropriate admission to the departments.

Responsibilities:

Communication

Demonstrates the highest level of communication and professionalism with patients caregivers rehabilitation staff providers and other health care professionals both within the academic medical center and in the community.

    • Exhibits effective and professional verbal and non-verbal communication
    • Communicates clearly respectfully and honestly with patients and/or caregivers and colleagues
    • Communicates proactively with other caregivers involved in patients� care
    • Adjusts communication and response to the level of understanding of the listener
    • Communicates status of pre-admission assessments with medical directors and coordinates admissions with rehabilitation unit leadership

    Resolves problems and concerns by serving as a liaison between referral sources and Brown University Health rehabilitation staff to improve service to the patient and the referring facility

    Communicates patient update and admission status to referring individual in a timely manner

    Informs potential and accepted patients and their caregivers about Vanderbilt Rehabilitation Center�s program and gathers and responds to input regarding their need for rehab information

    Informs Brown University Health rehabilitation leadership of issues and/or concerns in a timely manner

    Marketing

    Promotes services of Brown University Health rehabilitation programs to referring facilities discharge planners social service agencies providers patients family members and third-party payers

    Provides educational programs for referral sources

    Conducts/arranges tours of center for referral sources/family members

    Pre-Admission Assessment

    Considers age specific needs according to scope of services when planning implementing and evaluating care

    Identifies and demonstrates ability to assess patient�s status consider any special needs and interpret appropriate rehabilitation and medical needs in order to make appropriate recommendation for IRF level of care to rehab physician

    Screens all referrals for appropriate placement based on Medicare and other third-party payers guidelines and on the admission criteria established by Brown University Health rehabilitation leadership

    Financial

    Demonstrates knowledge of current rules and regulations related to third party reimbursement and their impact on rehabilitation admissions

    Confers with financial counselors on pre-admission financial screening

    Obtains pre-authorization for admission from third party payers as required

    Other Duties

    Provides demographic data and referral statistics on screening information to Brown University Health rehabilitation leadership for trend analysis

    Demonstrates knowledge of UDS and IRF-PAI to assure regulatory compliance

    Displays competency in Quality Indicator scoring

    Participates in departmental and/or interdepartmental quality improvement activities

Other information:

BASIC KNOWLEDGE:

Graduate from a school of nursing and licensed to practice as a Registered Nurse in the State of Rhode Island . BSN and certification in rehabilitation nursing desired. Alternate credentials include license to practice Occupational Therapy Physical Therapy or Speech Language Pathology in Rhode Island. Knowledge of utilization review process health care reimbursement and physical rehabilitation principles.

WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS:

Must be fit for duty in accordance with OSHA standards and program specific needs.
