Clinical Nurse Manager

Summary:

Reporting to the Medical Director the Clinic Nurse is a manager who oversees clinic services provided onsite at the Gateway three CCBHC locations through effective leadership and direction to medical assistant and clinic staff supply management interdisciplinary collaboration care coordination and direct care to individuals serviced. The Clinic Nurse is responsible for understanding and maintaining the scope and standards of practice to medical assistants work to the top of their license/certification. All essential functions performed in this position reflect the age specific needs of the patients. 

Responsibilities:

Ongoing support of a psychiatric care environment that is trauma-informed least restrictive environment that is free from coercion retaliation or discipline.

Supports a workplace culture of safety including team approaches to patient safety an atmosphere which promotes reporting of safety incidents and which places focus on medication safety communication and prevention of adverse outcomes.

Provides direct supervision and oversight to medical assistants and dedicated case managers assigned to CCBHC locations including managing work assignments training hours time off and performance evaluations. Monitors and evaluates the assignment of patient care to the appropriate personnel and level of care needed.

Evaluates the individual performance of each member of the staff and provides developmental feedback; assessment is on-going and reviewed annually. Meets with employees to discuss and evaluate performance and review individual performance goals at appropriate interval periods.

Responsible for developing employees to their fullest work potential by identifying educational opportunities and growth experiences.

Schedules regular staff meeting with the clinic staff for the purpose of providing the employees with day-to-day operational information quality and policy updates site information organizational news from the system and CCBHC leadership.

Ensures the CCBHC teams are kept appraised of patients� conditions coordinates resource serves as a liaison between the psychiatry team community health agencies primary and specialty care providers recovery centers and staff to deliver high quality individualized care.

Collaborate with the Medical Director CCBHC Directors and clinical pharmacist to ensure comprehensive clinic services are provided to meet client needs across all locations.

Provides direct patient care complex patients as identified or assigned. Completes primary care coordination and screening according to CCBHC standards. Documents patient progress compliance and other essential information in the medical record. Ensures patient�s care team primary care and/or sub-specialty providers are provided with timely information on patient�s progress and additional needs.

Ensures compliance with CCBHC service requirements per policy/procedure provides primary care screening and monitoring for both adults and children to include building relationships with community resources and contacts (i.e. primary care providers (PCPs) dentists specialty providers other hospitals treatment centers and state agencies) and assisting patients to gain access/referrals to those services/resources; educating patients/caregivers to assist with understanding illness treatment and follow-up.

Evaluates care provided by clinic staff in relation to the patient�s needs. Evaluates care rendered and measures against the care standards. Evaluates documentation of care.

Responsible for coordinating the daily operational activities required for the effective functioning of the area.

Maintains quality assurance safety environmental and infection control in accordance with established hospital department policies procedures and objectives. Collaborates with Director of Quality to ensure reporting of key health indicators and other identified metrics.

Monitors exam rooms medication and storage areas at the clinic to ensure compliance with the environment of care expectations. Maintains par levels of supplies. Collaborate with pharmacy laboratory and environmental partners on an ongoing basis to ensure the needs of clinics are met.

Collaborates across the CCBHC teams to ensure cross coverage of nursing duties ensuring exceptional patient care.

BASIC KNOWLEDGE:

Bachelors degree in nursing Master�s preferred. Combination of nursing degree and experience may be considered.

Must have current licensure to practice as a Registered Nurse in the State of Rhode Island or in another state that participates in the Nurse Licensure Compact.

Demonstrated knowledge and skills necessary to provide care to patients throughout the life span with consideration of aging processes human development stages and cultural patterns in each step of the care process. Understanding of treatment of individuals within community behavioral health and substance recovery care environments.

EXPERIENCE:

Two to Three years� relevant experience with demonstrated clinical and leadership skills.

INDEPENDENT ACTION:

Incumbent functions independently within a broad scope of department policies and practices; generally refers specific problems to supervisors only where clarification of departmental policies and procedures may be required.

SUPERVISORY RESPONSIBILITY:

Direct supervision for up to 10 full-time equivalent personnel. Supervision includes hiring orientation training and generation of personnel actions or recommendations.
